Check request forms are to be completed by all California Society, Sons of the American Revolution Officers and Committee Chairmen when seeking reimbursement for their expenses. Upon completion the form should be mailed with supporting documentation and receipts to the California Society president. Be sure to sign all check requests before placing them in the mail.
